Collect these items: Aadhaar Card (original + photocopy), 2002 voter list proof (if available), current Voter ID, and address proof (electricity bill, bank passbook). Without 2002 proof, don’t worry — BLO will verify via neighbors. Keep everything ready to avoid last-minute panic.
Write your full name exactly as in Aadhaar in the first box. Enter Date of Birth in DD/MM/YYYY format. In family EPIC section, fill Father’s, Mother’s, or Spouse’s Voter ID number only if known. Use ‘Not Known’ if unsure. This helps BLO trace family linkage.
Get your current voter list PDF from your BLO or download from our 2002 list checker. Copy your name, EPIC, father’s name, AC number, Part No., and Serial No. exactly as printed — even if wrong. This is your baseline record.
Write your correct name, address, and status here. If you shifted, mention new AC. If name changed after marriage, write new name. If alive and same address, repeat left column data. This column decides your final voter list entry.
Read carefully and tick all three:
Use blue ink. Women and elderly should use left thumb impression. Men can sign. If you are absent, any adult family member can sign on your behalf. BLO will countersign below.
Do not mail or submit online. Wait for BLO visit (Nov 2025 – Jan 2026). Hand over printed form with documents. Get acknowledgment slip. If missed, visit your polling station with form.
